Professional Designation Program


  • Global Logistics Specialist International Program Application Form: Download and complete the International Program Application Form. The application form is intended to be printed and filled out by hand. You may also complete the form on the CSULB online application portal here. If you need assistance completing either form, please contact our office at (562) 985-8424.
  • $150 application fee (non-refundable): Send a personal check drawn on a U.S. bank account, a money order, cashier’s check, or bank check from either a U.S. or non-U.S. bank account. Make checks payable to “CSULB.” Visa and MasterCard also accepted. This fee will also be used to send your I-20 by express mail (2-3 day delivery).
  • Completed Financial Affidavit for ALI Students form.
  • Bank statement: To obtain an I-20 for an F-1 student visa, enclose a bank statement with a minimum balance of $15,350 U.S. dollars or equivalent convertible currency. Acceptable documents include your or your sponsor’s bank account in the U.S. or your own country. Currency need not be stated in U.S. dollars.
  • Passport Information: Please copy the pages in your passport on which your name and date of birth appear.
  • Original TOEFL or IELTS Score Report
  • Filing Dates: If you are applying from outside the United States and wish to obtain an F-1 student visa, your application should arrive at our office at least two months prior to the start of the academic term. In-country applicants may submit their applications with appropriate documents up to one week prior to the beginning of classes.
